The Passport Application is quite detailed (three pages) and includes an affidavit of her birth provided by her mother Lucile Sabin Johnson. Lucile Sabin Johnson also gave her consent for her daughter to visit Europe with a Miss Maude Marrow.

Lucile was planning to sail on 21 June 1924 and would be visiting England, Switzerland, France, Germany, Italy, and Belgium.

There is a section on the passport application requiring a description of the physical appearance of the applicant. Lucile responded that her had a small mouth, straight nose, high forehead, and an oval face. She was seventeen years old and 5 feet 4 1/2 inches tall.

The application also includes the date and location of the birth of Lucile Johnson (20 June 1906 in Larned, Kansas) and where her father William Houston Johnson was born (Cadiz, Ohio).
